Thank you for your question. Unfortunately a breast reduction requires incisions that are simply unavoidable. I appreciate your concern for scarring based upon your history of keloids, and to this end I would recommend you consult with a board-certified plastic surgeon and discuss your candidacy for a breast reduction and your scarring concerns further.
A breast reduction requires an anchor incision to remove the excess skin and breast tissue, and boost the nipple areolar complex back up to where it should be. These incisions typically heal quite nicely but healing can be quite variable. To help prevent poor scarring, ensuring that there is no tension on closure is important and many plastic surgeons (myself included) also recommend using scar treatment creams post-operatively.

I have reduced the size of breasts with a small cut and just liposuction. It does not reshape the breasts but does accomplish reduction. I do offer SKYPE/FACETIME complimentary consultations.

Unfortunately, a true breast reduction which both reduces size as well as lifts and reshapes the breast mound is impossible without some scarring (lollipop vs. anchor). The NAC is repositioned and the surrounding tissue is coned inwards, creating a less ptotic (sagging) more youthful mound.
Liposuction can sometimes be used to reduce breast volume (but only the adipose component). This, however, does not address sagging and nipple position. It has the benefit of a lower scar profile, but emptying the breast envelope without reshaping will likely only worsen the appearance.
While, there is no way to avoid the scar, we do have means for minimizing keloid scarring including silicone sheeting, Kenalog (steroid) injections, and 5-Fu.
